A dog vaccination record book keeps every vaccine, date, lot number and next due date in one place, so you can answer a groomer, boarding kennel or new vet in seconds. The most useful ones also track vet visits, medications, weight and the details a pet sitter needs.

Here's what to write down, an example page, and the record books we make for dogs and cats.

What to record for each vaccine

Vet clinics keep their own records, but you're the one asked for proof at the groomer, daycare, dog park or new clinic. Write these fields every time:

Field Example Why it matters
Vaccine Rabies (1-year) Some places only accept specific vaccines
Date given 03/14 Starts the clock for the next dose
Next due 03/14 next year Most useful line in the whole book
Clinic and vet Main St Animal Clinic, Dr. Lee Needed if records must be requested
Lot or tag number From the sticker the vet provides Helpful if there's ever a recall or reaction
Notes "Sleepy for a day" Reactions to mention next time

Ask the clinic for the vaccine sticker or printout at each visit and tape it next to the entry.

Sample record page

Vaccine Given Next due Clinic Notes
Rabies Mar 14 Mar 14 (1 yr) Main St Animal Clinic Tag #4471
DHPP booster Mar 14 Mar 14 (per vet) Main St Animal Clinic
Bordetella Jun 2 Per boarding rules Main St Animal Clinic Needed for daycare
Leptospirosis Jun 2 Per vet Main St Animal Clinic

Which vaccines your dog needs, and how often, depends on age, health, lifestyle and local law. Your veterinarian sets the schedule; the record book just makes sure nobody loses track of it.

Our pet health record options

Pet Health Record Book paperback cover

The Pet Health Record Book (paperback, $9.99) is a 6 x 9 inch, 120-page book for dogs and cats. It has a pet profile (breed, birthday, microchip number, allergies, insurance), vet and emergency contacts, a vaccination record, vet visit records, a medications and preventives log, a weight tracker, a grooming log, a feeding routine and pet sitter instructions. It's printed and shipped by Amazon, which makes it an easy gift for a new puppy owner.

Prefer to print your own? The Pet Health Records printable ($5.49) has the same sections in 11 pages, in US Letter and A4. Print one set per pet and add pages as they fill up.

The printable is also part of the Family Life Admin Bundle ($15.99) with a newborn tracker, kids chore chart, meal planner and family recipe binder.

Family Life Admin Bundle Printable set Families organizing everything at once

How to set up a record book in 15 minutes

  1. Call or email your vet and ask for your dog's vaccine history. Clinics can usually send it the same day.
  2. Fill in the profile page: microchip number, breed, birthday, allergies and insurance.
  3. Copy the vaccine history with dates and next-due dates. Put the next-due dates in your phone calendar too.
  4. Add the emergency vet and poison control numbers on the contacts page.
  5. Write the sitter page: feeding amounts, walk times, quirks and where the leash lives.
  6. Keep the book with the leash or carrier so it travels to every appointment.

When you'll be glad you have it

  • Boarding and daycare check-in, which often requires proof of rabies and bordetella.
  • Moving or switching vets, when records can take days to transfer.
  • Travel, especially across state lines or by air, where airlines and destinations may ask for health paperwork.
  • Emergencies, when a stressed owner can hand over one book instead of remembering dates.

Tips for multi-pet homes

Use one book per pet so dates never get mixed up. Color-code with a sticker on the cover, and keep all of the books together in a basket by the door. If you use the printable, print each pet's pages on a different color of paper.

A fun extra for dog people

Frequently asked questions

What should a dog vaccination record include?

Record the vaccine name, date given, next due date, clinic and veterinarian, and the lot or tag number from the vaccine sticker. Add notes about any reactions. Keeping the next due date prominent is the most useful habit, because it's what boarding, daycare and groomers check.

How do I get my dog's vaccination records?

Contact the veterinary clinic that gave the vaccines and ask for a vaccine history or medical record summary. Most clinics can email or print it quickly. Copy the dates into your record book and keep the printout tucked inside the back cover.

Does the record book work for cats too?

Yes. Both our paperback Pet Health Record Book and the printable Pet Health Records are designed for dogs and cats, with a pet profile, vaccination record, vet visits, medications, weight, grooming, feeding routine and pet sitter pages that suit either.

How often does a dog need vaccines?

It depends on the vaccine, your dog's age and health, local rabies laws and lifestyle factors like boarding or travel. Your veterinarian sets the schedule. A record book helps you follow it by keeping every date and next due date in one place.
